Bilingual Case Manager Lafayette Clinic


Duties:
  • Provides client-centered, problem-solving, and solution-focused counseling for patients with psychosocial issues.
  • Provides health education and case management for OB and chronic care patients.
  • Responsible for self-management and follow-up for patients with depression and diabetes.
  • Provides crisis intervention with psychosocial issues, such as DV, child abuse and neglect, suicide ideation, pregnancy decision counseling, and other issues as needed.
  • Functions as group visit/health education staff member as needed, including self-management with patients.
  • Conducts hospital visits as assigned.
  • Lafayette: collaborates with Behavioral health Professional and Psychiatrist to provide behavioral health counseling as part of the Integrated Services Project.
  • Prenatal Plus Coordinator for the pod.
  • Initial assessment of all pod OB patients (1 page questionnaire)
  • Detailed intake on PN+ and high risk OBs.
  • Responsible for care coordination and follow-up with all high risk OBs.
  • Responsible for real-time data entry on pod diabetes registries.
  • Responsible for regular, ongoing management of pod diabetes and depression registries.
  • Responsible for community agency outreach, networking and resource development.
  • Maintains tracking card on all pod OB patients.
  • Responsible for final data collection on pod OB patients (termed -COTRACK-).
  • Pecos CMs: Real time data entry on OB database.
  • Responsible for daily CM patient schedules.
  • Lafayette: Coordinates, organizes, and implements holiday programs.
  • Demonstrates knowledge of the principles of growth and development over the life span.
  • Responsible for being timely with attendance.
  • Responsible for reporting to the site Mental Health Professional worker for
  • Prenatal Plus program requirements and clinical supervision of patient psychosocial issues.

    Skills / Requirements

    Education / Experience:
    1.Two years college with 5 years equivalent work experience in case management, crisis intervention and client-centered counseling OR BA degree in Human Services field.
    2.Experience in maternal child health preferred.

    Knowledge, skills and abilities:
    1.Excellent verbal and written communication skills in English and Spanish required.
    2.Ability to flourish in a team management system.
    3.Sensitivity to low income, ethnic minority community.
    4.Excellent organizational skills.
