We are currently seeking a Case Manager to join our outstanding team.
POSITION SUMARY:
Assists aging persons and disabled adults to find long term care alternatives which will maintain them in the least-restrictive and supportive environment of their choosing. Manages support services and monitors clients- care. Acts as an information and resource guide for clients, family members and other professionals. Assists in Single Entry Point program evaluation, outreach, and resource development. Performs within regulations, guidelines, and deadlines established by State agencies.
Skills / Requirements
QUALIFICATIONS:
1. Education/Experience - Requires a B.A. or B.S. in human services field. Some experience preferred in working with frail and/or chronically ill population. Strong client interviewing and assessment skills. Experience in providing case management services, including interviewing and counseling are preferred.
2. Knowledge/Skills - Skilled in establishing and maintaining effective working relationships with clients, co-workers, medical staff, and the public. Knowledge of
intake practices and assessment skills. Knowledge of community resources available to clients and their families. Skilled in identifying and recommending problem resolution. Ability to work independently with minimum supervision.
3. Must have reliable automobile and current, valid driver-s license and auto insurance for making required client visits.
