Job: Licensed Practical Nurse

Columbine Health Systems operating 4 and 5 star rated skilled nursing facilities by the Center for Medicare & Medicaid Services, has opening for LPNs.

Licensed Practical Nurse assesses the resident's level of functioning on admission, transfer, and discharge; assists the RN in the development of an individualized plan of a care for assigned residents; coordinates care for individual residents on a day-to-day basis; accountable for teaching residents new information; accountable for effective communication with residents/ families, care team members, and physicians regarding the resident's progress on the plan of care and response to treatment; participates in the orientation of new employees; documents care thoroughly and accurately; participates in new employee orientation; attends in-services and staff meetings as required; and participates in all external licensure and accreditation surveys conducted in the facility.

New graduates and experienced workers are encouraged to apply.

Must possess as a minimum a nursing degree from an accredited college or university and possess a clear unencumbered license from the State of Colorado as a Licensed Practical Nurse. Prefer one year of experience as a licensed nurse in a hospital, long-term care facility, or other related health care facility. IV certification preferred upon hire; required within 6 months of hire.
