St. Paul Health Center is a privately owned long term care facility located in the heart of Denver, across from Denver’s City Park and Zoo.
We will be unveiling our new unit, Parkside, this September. Parkside is a state of the art unit for skilled nursing care, specializing in services and environments to promote therapeutic healing in the wake of a hospitalization. The rooms and amenities are first-class, and the unit is fronted by a new, grand entrance.
Saint Paul celebrated its 25th anniversary in 2005! Residents, families, staff and community members toasted the anniversary in November with live bands, delicious food, and the first public unveiling of the Parkside plans.

Social Worker - Long Term Care

Responsibilities:
Play a central role in the team at St. Paul Health Center, 155-bed nursing home. This position will work with the social services team in coordinating resident services, discharge planning and completion of required documentation.

Qualified applicants will possess strong communication skills, organizational skills, and 2+ yrs exp relating to social services including mental health. Long term care, medicaid/care and CAC training exp a plus.
