Jobing Description
We are looking for a full-time Therapist to work on our Omnibus Budget Reconciliation Act (OBRA) team. The primary purpose of the OBRA team is to provide therapeutic services to consumers residing in area nursing homes, the population of which mainly consists of early to middle age consumers who reside in nursing homes due to their severe and persistent mental illness (SPMI). The Therapist will be responsible for providing these clinical services in the form of case management, crisis intervention, and individual/group/family therapy.


Duties and Responsibilities:
  • Provides clinical services to nursing home consumers utilizing Cognitive Behavioral therapies within a Psychosocial Rehabilitation model of recovery.
  • Assists consumers with activities of daily living, life skills, symptom management, and coping skills in order to facilitate maximum independent functioning.
  • Provides case management, crisis intervention, individual, group and family therapy.
  • Documents services provided and program activity according to Community Reach Center policies and procedures. Maintains confidentiality of consumer records at all times.
  • Participates in the development and implementation of consumer treatment programs.
  • Attends care coordination meetings with nursing home staff.
  • Attends and participates in Community Reach Center in-service education and training opportunities as well as meetings and special events.
  • May act as a liaison between consumers, families of consumers, or external facilities and agencies.
  • Completes individualized assessments and treatment plans according to Community Reach Center policies and procedures.
  • Provides clinical consultation, education and training to nursing home staff as needed.


Skills / Requirements
Requirements:
  • A Master's Degree in Social Work or a PhD/PsyD from an accredited university.
  • 1-2 years of experience working with severe and persistently mentally ill populations.
  • A valid Drivers License.
  • Status as a Licensed Clinical Social Worker (LCSW) or Licensed Psychologist (LP) in Colorado.

Preferred Qualifications:
  • Experience working with nursing home populations.
