Jobing Description
We are looking for two part-time Clinicians to work with the residents of Mesa House, a residential treatment facility located in Commerce City, CO.

The shifts available are:

(A)Saturday-Sunday from 2:00pm-12:00am (two ten-hour shifts - benefitted position).

(B)Saturday-Sunday from 8:00am-4:00pm (two eight-hour shifts - non-benefitted position).

Mesa House is an Assisted Care Facility which houses up to 10 consumers. Unlike the other houses in Community Reach Centers residential continuum, Mesa House is for long-term residential placement. The facility is comprised of consumers who at this point in their life cannot live independently, but do not meet criteria for hospitalization or habitation in a nursing home. All consumers at Mesa House have a diagnosed Axis I disorder, and the Psychosocial Rehabilitation Model is used in the course of providing therapy.


Duties and Responsibilities:
  • Provides case management, counseling, and other clinical interventions to residents and families as appropriate to the position. Prepares treatment plans, discharge plans, and follow-up care programs. Provides therapeutic crisis intervention and emergency services as required.
  • Coordinates care with residents, families of residents, Community Reach Center staff, or external facilities and agencies as appropriate to ensure continuity of care.
  • Participates in the review of clinical issues and program policies and procedures.
  • Monitors Activities of Daily Living (personal care, food preparation, laundry, etc.) for residents and provides training as needed.
  • Works with residents in the use of basic community resources and case management. Refers, links, advocates, and acts as liaison when necessary.
  • Assists in the day-to-day operation of the facility under the direction of a supervisor.
  • Assists residents with medication administration as directed by Colorado regulations, facility policies and procedures, and physicians orders.
  • Must be able to drive agency van or company car and transport consumers when needed.


Skills / Requirements
Requirements:
  • Bachelor's Degree in Psychology or other human services related field.
  • Must be a team player with good communication skills.
  • Knowledge of severe and persistent mental illness, cognitive/behavioral psychology and group/individual therapy.
  • Familiarity and understanding of MS Office applications, particularly MS Outlook.
  • Requires a valid Driver's License.

Preferred Qualifications:
  • Experience working with mentally ill populations.
  • Experience working in residential care facilities.
