Jobing Description

We are looking for a full-time bilingual Therapist to work with our Home Team, which provides in-home stabilization and crisis management for at-risk families and individual consumers in Adams County. Under supervision, it is the Therapist’s responsibility to provide direct mental health clinical assessments and treatment to consumers and/or their families. This position primarily operates in homes, but may also operate in a residential environment, or in an outreach environment (such as schools and/or outpatient clinics).

Duties and Responsibilities:

Provides mental health treatment services through a community-based delivery model to consumers and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and emergency services as required.
Provides care to consumers in a manner that promotes safety, comfort, and the therapeutic environment.
Maintains confidentiality of consumer records.
Serves as official care coordinator and clinician of record for caseload.
Provides case management services to consumers as needed.
Provides psychosocial rehabilitation services to consumers as appropriate.
Provides individual therapy, group counseling, family therapy, and other clinical interventions as appropriate.
Attends and participates in group sessions, in-service education and training opportunities, meetings, supervisory sessions, and special events.
Remains available to assigned families by pager during scheduled hours.

Skills / Requirements

Requirements:

Minimum of a Master’s Degree in Psychology, Social Work, or a directly related field.
At least 6-12 months of direct clinical experience, which may include internship/practicum experience.
Ability to perform therapy in Spanish.

Preferred Qualifications:

Professional counseling licensure (LP, LPC, LMFT, LCSW).
