Jobing Description

We are seeking a full-time Therapist to work in the School-Based Health Clinic located at the Adams City High School Wellness Center in Commerce City, CO. Under supervision, the Therapist will provide direct mental health clinical assessment and treatment to consumers and/or their families by using evidence-based practice models. This position is primarily located in the outreach environment of the school, but may also function in residential environments, homes, outpatient clinics, or primary physical healthcare sites.

Duties and Responsibilities:

Provides one-on-one assessment, consultation and brief intervention for psychological and behavioral health problems in a manner that promotes safety, comfort, and the therapeutic environment.
Provides individual, group, family, and case management interventions to students seen at the School-Based Health Clinic and/or their families.
Provides effective treatment planning and assistance to clients in coordinating care with primary healthcare providers.
Maintains confidentiality of consumer records.
Evaluates crisis situations and implements appropriate interventions.
Provides substance abuse prevention, screening, and intervention services.
Actively participates in meetings that support an integrated healthcare model.
Teaches patients, families, school personnel, and healthcare staff the care, prevention and treatment techniques of psychiatric disorders.
Assists the primary healthcare team in developing and implementing care management processes, such as the use of guidelines, disease management techniques, case management and patient education.
Monitors the site’s behavioral health program, and identifies problems related to patient services and makes recommendations for improvement.
Ensures referrals to Community Reach Center programs for ongoing services.

Skills / Requirements

Requirements:

Minimum of a Master’s Degree in Psychology, Social Work, or a directly related field.
At least two years of direct clinical experience in assessment, clinical service delivery, case management, treatment planning, and discharge planning.
Excellent working knowledge of behavioral medicine and evidence-based interventions for mental health conditions in a primary care setting.
Comfort with a primary care pace and working with an interdisciplinary team.
Ability to design and implement clinical pathways and protocols that are evidence-based in a primary care environment for behavioral and mental health conditions.

Preferred Qualifications:

Professional counseling licensure (LP, LPC, LMFT, LCSW).
