Job Details:
This position is for the Mobile Assessment Team. Clinician will respond to crisis calls in the community, with police, at the high schools, CSU, urgent care centers etc. Must be willing / able to do very quick triage assessments, crisis stabilization, and outpatient referrals.Summary: Provides triage, assessment, treatment, resource information and referral, education, utilization, and disposition planning to individuals, families and significant others, and groups. Functions as part of an interdisciplinary team including physicians/psychiatrists and other medical providers to develop individualized treatment plans, provide case management and disposition. Services are provided using accepted psychotherapeutic models and interventions, including integrative healthcare options appropriate to the patient`s diagnosis.Participates in initiatives and programs related to the vision, mission, and values of PVHS. Understands and represents the strategic goals of service lines as assigned. Develops and maintains knowledge and understanding of medical and/or psychiatric disease processes, treatments, and emotional issues related to these diagnoses. Coordinates psychosocial care and interfaces with third parties and community agencies throughout the continuum of care.Demonstrates proficiency in customer relations with clients and community systems. Initiates 27-10 holds and Emergency Commitments as appropriate to patients` situations.

Requirements: Master`s Degree in Social Work, Counseling, Psychology or a related field. Previous LCSW, LPC, or LMFT experience in hospital preferred. One year LCSW, LPC, or LMFT experience preferred. Licensed Clinical Social Worker in the State of Colorado.or Licensed Professional Counselor in the State of Colorado. or Licensed Marriage and Family Therapist in the State of Colorado. or Licensed Social Worker with planned advancement to Licensed Clinical Social Worker with identified Board of Social Work guidelines.
