Provide brief behavioral health interventions to MCPN clients

• Provide behavioral consult and direction to MCPN providers about clients who have psychiatric illness and/or other psychosocial issues interfering in their primary care treatment. Work as a team member with in the medical model
• Assess MCPN patient psychosocial, psychiatric and behavioral health needs and provide 1 to 2 sessions focused treatment. This may include but is not limited to psycho education, teaching communications kills, clearing up misunderstandings between providers and patients, making referrals to other agency for more intensive treatment, assisting in resolving funding issues, working within the patient’s culture to resolve barriers to treatment, motivational interviewing/working to elicit change behavior.
• Working with MCPN providers to refer patients with covered diagnoses or severe psychiatric illness to JCMH when needed.
• When needed, working with MCPN patients address substance abuse/addition, again, working with Motivational Interviewing to stimulate change. Make referrals to outside substance treatment facility when necessary and working with provider and patients to minimize abuse/addiction to prescription medication.
• Documentation of all encounters in the MCPN chart.

MSW/LCSW,LPC, Psy D, Licensed Psychologist, MSN, preferred also to have a CACII or III. Bilingual strongly preferred.
