COMPLETE HOME HEALTH CARE is a Medicare/Medicaid certified home care agency serving clients up and down the front range with branches in Boulder, Denver, S. Denver, Brighton, Longmont, Colorado Springs, Canon City and Pueblo.

Home Care Services include skilled nursing, rehabilitation, homemaking and personal care, wound care, diabetic teaching, cardiac/pulmonary care, pediatrics and home phototherapy. Our Home Care Team is comprised of a Medical Director, an Advisory Committee, Clinical Care Coordinators, Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, Certified Nurse Aides and Personal Care Providers. CHHC is a member of the Better Business Bureau.

We are currently seeking an Experienced Personal Care Provider.
Responsibilities of Personal Care Provider includes:
Provides personal care and/or homemaking services to clients under the direction of the Personal Care Coordinator and the established Plan of Care.
Completes & submits accurate documentation of visits, reports changes in client conditions to the Coordinator or Clinical Supervisor, abides by OSHA and agency rules for infection control & safety.
Participates in agency inservices, after-hour, weekend, and on-call duties as requested.
Skills / Requirements
JOB REQUIREMENTS
Able to participate in entire orientation process & pass skills tests successfully
Able to follow personal care and/or homemaking plans of care
Able to travel to clients homes in all weather conditions
Able to manage time efficiently
Able to write or type legible progress notes, read, and be able to follow instructions
Able to promote strong interpersonal relationships and deal with the demands of the position
Able to demonstrate effective oral and written communication skills
Able to lift up to 50 lbs
Able to hear and observe client during treatment, as well as voice instructions clearly
Able to work in environments that may be unpredictable
Able to walk or navigate.
