Total Longterm Care is a non-profit PACE program that provides managed health care and supportive services designed to assist seniors who want an alternative to nursing home placement. With Total Longterm Care's assistance, participants can stay in their own homes and communities. The mission of Total Longterm Care is to enable frail individuals to live with dignity in their communities.

We are currently seeking a End of Life RN to join our outstanding team.

POSITION SUMMARY:

Performs initial End of Life assessments on participants in facilities and home
environment (if workload allows) to determine their individual needs and coordinate plan of care with care-giving resources. On an ongoing basis, reviews plan of care, re-assesses participants‚ changing needs, and evaluates how those needs are being met by TLC and facility staff. The main objective of this position is to support provision of high quality of care for EOL participants with a focus on minimization of unpleasant symptoms/pain, provide psychosocial support, and insure highest level of comfort through this transition. Oversees appropriate medication regimen, maintains participant‚s medical record with thorough documentation, provides education and counseling to participants, facility staff and families, attends facility participant conferences, morning, and EOL meetings as indicated. Assists with organizing and presenting EOL in-services to TLC and facility staff. Responsible for meeting TLC licensure requirements.

Skills / Requirements

QUALIFICATIONS:

1. Education - Graduate of an accredited nursing program, current Colorado
Registered Nurses License, CPR certification.

2. Experience - Two years of health care experience with emphasis in geriatrics, One year of EOL/Hospice experience.

3. Skills/Knowledge - Broad knowledge base of health care problems of the frail elderly. Knowledge of common safety hazards and precautions to establish a safe working environment. Skilled in identifying problems and recommending solutions. Knowledge of home care services, and nursing principles and practices, with particular reference to the elderly. Ability to conduct end of life assessments and develop appropriate plans of care. Knowledge of supportive/bereavement counseling techniques. Skilled in preparing and maintaining records and writing reports, and responding to correspondence. Excellent organization and documentation skills. Familiarity with bereavement/coping strategies and resources that are available to participants and families. Skilled in developing and maintaining department quality assurance. Skilled in establishing and maintaining effective working relationships with facility staff, participants, medical staff, TLC staff members and family caregivers. Excellent customer service skills to support effectiveness in a variety of metro-area-facilities. Ability to react calmly and effectively in stressful situations. Current Colorado Drivers License, reliable transportation, satisfactory driving record and proof of auto insurance.
