Position exists to provide non-emergent off-site and on-site in home assessments and psychotherapy for Senior Reach referrals who reside in Jefferson, Clear Creek, Gilpin, and Broomfield Counties as indicated by the Denver Regional Council of Governments (DRCOG) grant specifications. This is to be accomplished by working as a team member with Senior Reach staff from the Mental Health Center serving Boulder and Broomfield County, the Seniors‚ Resource Center, as well as from Jefferson Center. The DRCOG Outreach Clinician is a representative for Senior Reach internally and externally and works with the project team in all aspects of coordination, training, and recruitment efforts. Extensive day travel is required.
* Provides individual in home assessments, individual counseling services based on a strength-based model and develops individualized strength-based treatment plans. Engages consumer and family in the treatment process and goal-setting. Identifies risk factors including lethality and responds to barriers to successful treatment. Works constructively with consumer to reach agreed upon outcome, and coordinates care with internal and external providers. Maintains appropriate professional standards and provides appropriate follow-up for clients.
* Completes Senior Reach and SAMS documentation in an accurate and timely manner and assists with data collection to fulfill DRCOG grant requirements.
* Acts as a primary representative for Senior Reach and arranges and conducts outreach presentations and training on older adult issues. Conducts depressions screens in the community.
* Meet required number of consumer service hours as determined by Network Director.
* Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.
Skills / Requirements
LPC, LCSW, LMFT or comparable license plus two or more year‚s related experience and/or training with older adults. Bilingual/Spanish preferred. CAC preferred. Must have strong knowledge and ability to work effectively with the brief solution-focused treatment model.
Experience must be a combination of behavioral health administration, clinical practice, or other clinical experiences that provide the tools necessary to successfully fill this position. Position requires experience with relevant computer applications including electronic medical records, passion and understanding of older adult issues, caseload management skills, good writing skills and presentation/teaching experience and skills. Ability to maintain a high level of efficiency and compliance with administrative rules and standards. Strong knowledge base of service community, agencies and providers who work with older adults.
Must have reliable personal transportation. Flexible work hours may be required occasionally.
