Performs patient observation, data collection related to patient care, administers medication, treatment and proceduresaccording to the physicians' orders and resident care plan. Communicates patient findings to the supervisor. Assumesresponsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify andmeet the physical, emotional and spiritual needs of the patients. Oversees care provided by CNAs, Care Partners, QMAPsand/or other support staff. Promotes the optimal health, well-being and safety of the patient through use of the nursingprocess and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personalaccountability for relationship-based care, organizational mission, and core values.
What You Bring/Job Requirements
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
One year of previous LPN experience, preferredRequires knowledge in nursing and person-centered care conceptsAbility to demonstrate competency in the provision of individual care of patients based on the patients' assessed age anddevelopmental needs.
Current state LPN license for applicable state of employment or current LPN license from a participating statein National Compact Licensure (Colorado only)
Shifts: 7a-730pRotate weekends, approximately every 3rd weekend
Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
