Job Description

Shift:  Monday through Friday, 7:30 a.m. to 4:00 p.m.

OBJECTIVE: 

To develop and implement a Residential program for all assigned participants to achieve their individual goals To provide intensive support services to develop the abilities necessary to maximize their independent functioning in a residential and community environment.   

QUALIFICATIONS: 

Bachelor’s Degree in Vocational Rehabilitation, Psychology, Sociology, Counseling and Guidance or related degree. Applicable field or equivalent combination of training and experience in a social service, counseling or rehabilitation setting, providing direct services to persons with physical, emotional and/or developmental disabilities will also be considered. A valid Colorado driver’s license, vehicle insurance, and willingness to use personal vehicle for community work is required.

OTHER REQUIREMENTS/PREFERENCES:

It is preferred that the incumbent have excellent written and verbal communication skills and demonstrate the ability to interact with various types of internal and external customers. It is also preferred that the incumbent have the ability to relate effectively to people with disabilities, their parents, host home providers and guardians and be empathetic toward their needs. The incumbent must have the ability to work independently, cooperatively and to exercise effective planning. The Case Manager must demonstrate flexibility within the framework and pressure of established deadlines. The incumbent must be able to write clear, concise and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills. The incumbent must have a working knowledge of community resources, have the ability to program, train and be creative in motivating Participants toward individual success. The incumbent must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.   The Case Manager must have experience speaking effectively to various small groups in a presenting/informative role.

  Overtime may be required as well as some travel to attend meetings and required training. The ability to work occasional weekends and nights for the Residential on-call system is required.  

KEY RESPONSIBILITY AREAS    

Case Management

The Case Manager will provide direction, supervision, and case management for all assigned participants. The incumbent will complete all assigned paperwork required through the on-line Participant database system. Examples of this paperwork include: participants plans, residential home visits, medication and appointment oversight etc. It is critical that the Case manager have the ability to encourage independence by assisting participants in problem solving their personal and program related problems by using accepted and innovative rehabilitation techniques and maintain a proactive environment. In addition, the incumbent will teach proper social habits, interaction skills, and self-help skills through role modeling and programmatic intervention. The Case Manager will monitor participant activities and facilitate Participant specific assistance where necessary. The incumbent will assume total responsibility for each participant’s file and ensure all state standards are adhered to.

Team Support 

The Case Manager will provide residential providers with extensive, person-specific information regarding the participant so problems and concerns can be handled effectively. The incumbent must ensure all providers are provided with a copy of the individual program plan as well as necessary tracking information. The incumbent will attend all IDT and Human Rights committee meetings representing Discover Goodwill in a professional manner. The case manager will act as the central communication hub for all participants, providers and internal team members. Through this responsibility, the incumbent will ensure pertinent information is shared and work as a cohesive team member to ensure complete health, safety and happiness occurs with each participant. The case manager will be responsible to participate in home visits, complete required paperwork regarding these visits, and complete all follow up needed. These home visits can be announced or unannounced and may be in conjunction with additional team members. The case manager will function as an active member of the Residential on-call system and be seen as an expert in handling Participant incidents during non-work routine hours.

Participant/Program Tracking and Reporting 

The Case Manager must complete all necessary documentation, e.g., incident reports, financial information, Social Security and Medicaid eligibility, monthly billing and medical information.   It is critical that the incumbent maintain complete and accurate records and statistical data as required for all programs. The case manager must be comfortable with using a computer based data system and will be responsible to help train all providers on the system. The incumbent will also be responsible to ensure all provider contractual paperwork is complete to include: initial and annual contracts renewal, timecards and monthly invoices.

Relationship Management  

The Case Manager will communicate and coordinate with other involved agencies and support personnel, e.g., day program staff, quality improvement staff, The Resource Exchange case management, parents, guardians, and therapists. The incumbent will keep the Program Coordinator informed of progress, considerations, and pertinent information. The Case Manager will deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required by the Residential team.
