PRIMARY RELATIONSHIPS: Center Manager, Executive Director, Center staff, residents, family members and public.
OBJECTIVE: Under the supervision of the Center Manager, manages flow of clinic leading all staff in provision of medical and nursing care, ensuring high quality standards. Coordinates services with primary care team, acts as primary liaison. Triages and assesses participants as needed. Acts as gatekeeper for daily clinic scheduling. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family and staff teaching and communicates participant's condition to caregivers, primary care and the interdisciplinary team.
1. Conducts an in-person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive care plan for each participant.
2. Conducts an in-person nursing re-assessment semi-annually and as needed.
3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on plan of care planning decisions and coordination for twenty-four (24) hour care delivery.
4. Provides orientation, initial and annual competencies, supervises and evaluates performance of Clinical Registered Nurse (R.N.), Licensed Practical Nurse (L.P.N.), Certified Nursing Assistant (C.N.A.), and Clinic Clerk. Has indirect supervision of Day Center Workers.
5. Performs competencies on appropriate Day Center staff as needed.
6. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Ensures that clinic staff works within their scope of practice.
7. Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinic staff to function independently.
8. Evaluates the overall effectiveness of the clinic, implementing change and quality improvement as needed.
9. Manages flow of clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensures timely and appropriate care of participants.
10. Takes lead in effectively communicating with Interdisciplinary Team community agencies at meetings and throughout the day.
11. Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders.
12. Coordinates participant care with outside contracted service providers - nursing homes, assisted living, lab, oxygen, etc.
13. Communicates with weekend and after hour staff, following up appropriately.
14. Manages and reviews participant's records, charts, pertinent information and staff's documentation. Documents as needed in medical records according to Senior CommUnity Care standards.
15. Performs general skilled nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination and testing of participants.
16. Responds to and perform Triage of incoming telephone calls. Oversees scheduling of outside appointments for participants when necessary.
17. Oversees scheduling specialist appointments and assisting in hospital admissions and calling report to unit.
18. Educates participants, family and caregivers in regards to care, medications and treatments.
19. Educates staff in regards to care and treatment for the elderly as needed.

21. Monitors clinic department expenditures and meets budgetary expectations.
22. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
23. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
24. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
25. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
26. Participates in and supports Quality Improvement initiatives.
27. Performs other duties as required or requested.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
2. Must have a valid driver's license, proof of insurance and have means of transportation.
3. Education: Graduate of a school of professional nursing program required. Bachelors of Science Registered Nursing (BSRN) strongly preferred. Current Colorado Registered Nurses License and Cardio Pulmonary Resuscitation (CPR) certification required.
4. Experience: A minimum of one year's experience in working with the frail or elderly population required. Two (2) years of nurse management experience in a hospital, nursing home or community-based setting preferred.
- Management and leadership skills.
- Broad knowledge base of health care problems of the frail elderly.
- Skilled in physical assessment and triaging.
- Knowledge of medical equipment and instruments.
- Knowledge of common safety hazards and precautions to establish a safe working environment.
- Skilled in identifying problems and recommending solutions.
- Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
- Ability to react calmly and effectively in emergency situations.

1. Physical Requirements - Requires full range of body motion including handling and lifting patients. Requires standing, walking, pushing, bending, kneeling and reaching at arm's length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.
2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants' service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers.
3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that may have the potential for verbal or physical aggression.
